I wholeheartedly agree on the movie metaphor.  Pretty much all sociopaths are lacking in empathy and theory of mind so their worlds invariably become solipsistic in nature.   But the true malignant narcissist creates a storyline like a darker, twisted Walter Mitty screenplay to pour in all the emotional conflict and drama.  Obviously there’s no shortage of Trump examples but the one that consistently comes to my mind is when Bill Barr forcefully challenged his fanciful distortions of election fraud to which Trump kept muttering, as much to himself as a deux, “You must hate Trump.  You must hate Trump.”
