I think it may have been Arsenic and Old Lace with Cary Grant and in the story, Grant is trying to get his brother locked up in a mental institution and he calls one up and tells the doctor he wants to put his brother in there and the doctor asks what is going on with him. Grant tells him his brother thinks he is Teddy Roosevelt and the doctor says "Oh no, no, I can't take a Teddy Roosevelt, I already have one. That would be a disaster!"
