It's a great question, Andi, and I think you are looking at it very accurately.  I think it gets to the heart of what is the essence of narcissism.  If I had to try and slice the pie I would say there is a cognitive component characterized by an absence of theory of mind, empathy, and perspective taking and an emotional component where the narcissist has an inherently unstable sense of self so they respond dramatically to rejection, lack of attention, or lack of appreciation.  Both halves seem to create that whole.
