I just watched this last night and greatly enjoyed it.  It is a dark, gritty urban tale with Sean Penn and Tye Sheridan as paramedics in NY City. It is a contemplative film with fine acting mostly throughout the film (though it is clear - former boxer Mike Tyson can't act worth a shit).

Penn plays a bone-weary medic with an exterior so worn and leathery that you wonder if anything could pierce it.   Sheridan plays a rookie paramedic struggling with the inhumanity of what he sees while spiraling further downward in his search for how to cope with urban horrors often found in everyday life.

If I had to find an apt comparison, I would suggest imagining the blending of urban and large city cinematography by Sidney Lumet, Bringing Out The Dead, Taxi Driver, and other stylistic aspects found in other movies by Paul Schrader, it would be close to this. 

The filming is spectral at times, as well as moody and melancholy.
