Crappy headphones will cover up issues with low bit-rates, but listening through a good set of phones, I can definitely tell the difference between a CD and a 256k (or even 320k) MP3. There's a fullness that is missing with the MP3, particularly in the percussion and reverbed-up guitars.I've done side-by-side comparisons with lossless formats like FLAC and it is much more difficult to tell the difference.
