Tiered Health care will be a non-starter for a LOT of people.
One of the options being discussed is to let people opt-in to Medicare or their Company insurance. This would greatly increase Medicare's funds/pool and effectively offer stiff competition to the insurance companies to reduce costs.
Another option is to go Kaiser's route - employ Salaried doctors in a comprehensive health care system/pool. My sister has had Kaiser for years and she loves it. Hasn't paid a deductable and has never been refused a treatment.
