ABC News is reporting NH changed the procedure for placing peoples' names on ballots this year and that this may have added to Clinton's win.

Still though, it doesn't fully explain the phenomenon of how some polls put Obama up by 13 only to lose by 3. (I'm thinking one thing might be that HRC was gaining fast on him and it wasn't being reflected in the later polls and he was either staying stagnant or dropping.)
