My own interpetation was much less prosaic - but Tony dies, none the less.
First, the reference to "You never see it coming"
Second, in the Parlay scene, they ask for Phil's location & are refused. Little Carmine looks squirlly about the whole thing.
Third, they track Phil down with the help of the FBI agent and whack him in front of his wife and grand children - a massive betrayal of mob ethics, you never kill in front of the family
Fourth, the guy in the members only jacket goes into the bathroom (Reference to the godfather movie)
As the bell rings and medow enters, the members only guy comes out of the bathroom and shoots Tony. Screen goes black - Little Carmine ordered the hit, and is now the boss.
